Because Horizons is an enrichment program, we rely on teacher evaluations to provide input on each new student.s school performance. If you teach in the City of Rochester and would like to refer a child to the program, please review the Admission Requirements and complete a
Teacher Evaluation Form for that child. Upon receipt of the evaluation form, Horizons will determine eligibility and contact the child's family to begin the application process.
